Our media player is not connecting to the network, something isn't working right with the media player, or we need some other support with the equipment/network.

We need to add, change, or remove a media player at our facility and/or update contact information for our host, facility, or media player, and/or add, change, or remove physicians related to our facility.
NOTE: All location/facility change requests will be reviewed upon submission and verified with the proper administrator/representative.

Content CHANGE Ticket: submit a request for existing content to be altered, started or stopped on different dates, etc...or any other content request that is not NEW Content.
